COVID-19 Symptomatic Screening Form

The surgery is currently open for booked appointments. However, please do not attend if any of the following apply to you:

  • You have symptoms of Covid-19, including cough, fever, sore throat, muscle aches or headache.
  • You have been abroad in the past 14 days.
  • You have been in contact with a confirmed case of COVID-19 in the past 14 days.

If you develop symptoms of COVID-19, please self isolate immediately and contact us to arrange a COVID-19 test. This can be done by filling out the online submission form here. Once complete, a member of staff will contact you with further instructions. Alternatively, please contact the surgery reception.

COVID-19 Vaccination Clinics 

We are currently vaccinating all patients over the age of 70 years.

As of 15th April 2021, the HSE have advised us that all patients between the ages of 60- 69 will be vaccinated in the HSE-led COVID-19 Vaccination Centres. This applies to all patients in this age group, irrespective of whether or not you have an underlying health condition. If you are in this group, please register for your vaccine on the HSE website as directed. You can access the HSE website vaccination portal by clicking here.

In May and June, we will vaccinate Very High Risk (Group 4) and High Risk (Group 7) patients between the ages of 18 - 59. Patients who are registered with the practice, and who have any of the conditions listed below, are invited to contact the practice and register for our Group 4 and Group 7 COVID-19 Vaccination Clinics. Please note, even if you are not eligible for vaccination in these clinics, the HSE have reassured us that most adults will likely be offered a vaccine in a HSE-led COVID-19 Vaccination Centres by the end of June. We therefore ask for your patience at this time.   

Group 4 (age 18 - 59):

  • Diabetics with a HbA1c > 58 mmol in the last 12 months
  • Obesity BMI > 40
  • Chronic Respiratory Disease on home oxygen or hospitalised in the past 12 months

Group 7 (age 18 - 59):

  • Diabetics all other diabetics Type 1 and 2

  • Obesity BMI > 35

  • Chronic Respiratory Disease severe asthma or COPD (i.e. requiring frequent or continuous use of oral steroids)

  • Chronic Heart Disease – this includes patients with heart disease, vascular disease and heart failure 

  • Immunocompromised

  • Cancer - diagnosed in the past 12 months

  • Chronic Kidney, Liver or Neurological Disease

If you have any of the above conditions, please contact the practice to register for our Group 4 and Group 7 COVID-19 Vaccination Clinics.

Please note, patients who register who do not meet the eligibility criteria will not be offered a vaccine. These patients, however, will likely be offered a vaccine in a HSE-led COVID-19 Vaccination Centre by the end of June.

If you register and are eligible to receive a vaccine in one of our Group 4 and Group 7 COVID-19 Vaccination Clinics, we will contact you 1-2 days before your appointment. Please be advised that vaccination is not guaranteed even if you register and are eligible. This is because vaccine supply is not guaranteed. We therefore would like to encourage Group 4 and Group 7 patients to accept earlier invitations for vaccination elsewhere, should they receive them.
